[Board] community survey

Good morning,

Once upon a time there was an annual community survey done to gather
information about the Evergreen community.  This hasn't been since 2017, I
believe, but I'm interested in starting it back up and making the data
available again.  I wanted to get a feel for the room in terms of support
for doing this and having it come from the board.  If there is interest I
will propose a set of questions based heavily on the last one we used as a
starting point for discussion.
