[Board] community survey

The three questions I'm looking at adding are

1) Operating budget.  This is pretty commonly used and for many public
libraries a matter of public record.  It is optional though so that those
who don't want to can choose not to respond.
2) A question asking if they are okay with their information being
publicly accessible, say on the WIKI.
3) A question asking if they will allow us to follow up with any additional
questions.

Thoughts?
