[Evergreen-general] STAFF INITIALS-new accounts

We created a local solution to this circa 2012. We store a creator and 
create_date in actor.card, so we can see who issued each library card. 
It doesn't capture every edit, but does show who created the account 
initially and who issued any replacement library card(s).

I've added details to the noted bug.
