<p>Thank you for the interest in the form! To answer a few of the
questions:</p>
<p>- The form populates a pending patron in the pending patron
interface.</p>
<p>- The uploaded images are automatically deleted after a set
period of time.</p>
<p>- We had a great deal of discussion about the fact that we have
the digital images of the ID, and we consulted with our City's
lawyer about this. In the end, we went forward with it,
recognizing that patron interaction with the form is a type of
implied consent for us having access to these images. We only keep
the images for a set period of time, and there are two levels of
security for staff to be able to access them. We also offer other
options for obtaining a library card if a patron is uncomfortable
uploading their images with us. <br>

building an online form that allows us to capture the
cosigner information: <a href="https://www.grpl.org/get-a-card/" rel="noreferrer" target="_blank">https://www.grpl.org/get-a-card/</a></p>
<p>One of the nice features of the form is that it
allows a co-signer to register for multiple juvenile
cards at once. It also allows users to upload identity
and address documents so that we can then issue and
mail the cards.</p>

tried adding that field to your
registration page on the development
version of your site, but the change
caused a system failure when we
tried to create a patron. We looked
into the reason why we were unable
to add this field and here's what we
came up with:
<p>When someone goes to register a
card through this interface, the
results are sent to a database
table called "User Stage". This
table closely mirrors the "User"
table so that when a staff member
registers the "User Stage", the
data is dumped directly from this
table into the "User" table.
However, updates to the "User"
table don't always make it onto
"User Stage", and that is the case
with the guardian field. The only
way to get this to work properly
would be to change the schema of
the database to add this piece of
data on the "User Stage" table. "</p>

<p>This is surprising to us. We are
not a public library, and have had
few requests to register juveniles
so far. But since Evergreen was
designed for public libraries, we
were quite surprised to find that
we can't easily add the
Parent/Guardian field to the OPAC
registration form.</p>
