Hi Amy:

Pretty nice! The one area I found a bit confusing was the section around
"Primary contact": the "Evergreen responsibilities" seemed to be aimed
at the individual contact, rather than the whole institution, and I'm
not sure if I added more contacts whether they would also get their own
"Evergreen responsibilities" section.

If the responsibilities are meant to be associated with individuals
rather than the institution being described, I would be inclined to
leave the "Evergreen responsibilities" (if it is intended for the
individual contacts) associated with that person's RSCEL account, rather
than in the context of the institution. When you follow the link from
"Dan Scott" in the Institution Profile to
http://staging.rscel.org/node/1444, that page could list my
responsibilities; or they could be pulled in and displayed dynamically.

Also, we might want to move the "Web site" up right under the
"Description" section, to ensure that there's no momentary confusion
about whether the person filling out the form should link to
"http://projectconifer.ca" (the institution site) or
"http://coffeecode.net") (their personal site); similarly, it would
differentiate any confusion that a reader of the page would have about
the relation of the Web site to the person vs. institution. Moving it up
higher probably makes sense from a most-frequently-requested piece of
information perspective, too.

One other thought: along with operating system, we might want to get to
get operating system versions. Ubuntu 8.04 is very different from Ubuntu
10.04. It would be super-nice if the user was able to enter it in
free-text, with some semi-controlled choice made by autosuggest
providing the list of already-added options for that Operating System. 

(So if I choose "Debian", the version field should show me "Etch (4)"
and "Lenny (5)" from an autocomplete widget, but I should be able to
type "Squeeze (6)" if I'm a bleeding-edge type and that will establish
the entry for people who follow in my wake).
