At Conifer we're just following the steps that Mike and I wrote up at
http://open-ils.org/dokuwiki/doku.php?id=evergreen-admin:sru_and_z39.50
- although at Conifer we're running something closer to trunk than
1.4, so your mileage may vary slightly.

I spruced up the page a bit to document the options to limit search
scope and expose holdings that are supported as of Evergreen 1.6.
