[OPEN-ILS-DOCUMENTATION] How do you make a2x PDF output stylish?

Hi all,

The official docs has a really lovely PDF version with Evergreen
colors and a footer that asks folks to report doc bugs in launchpad.

However, when I run the a2x command, I just get the run-of-the-mill
dblatex style. [2, 3]

How can I get the lovely Evergreen formatting?  Is that defined in a
file somewhere?  Or are there more arguments I should be passing to
the a2x command?
