[OPEN-ILS-GENERAL] Check out issue

This issue was happening in PINES constantly, but (as far as we have seen) this was fixed in the 1.4 branch (we're running and we have not seen the problem in the last 9 to 10 months.  Perhaps the fix was PINES-specific, but I believe it was incorporated into trunk.  There is not an easy workaround, as far as we could tell.  We had our folks continue to check the item in until all instances of circulations were off the record.
