My library has been using Syndetics for years. They supply covers for all media types (even video games, though we haven’t tested that one). We have them supply covers for books and movies, which are quoted separately.
Syndetics is very responsive when I find the occasional cover that doesn’t match the item we have on hand. Fixes are usually made within a day.

At Berklee College we use Syndetics, which includes their audio/video covers for our extensive music CD and DVD collection. I believe the price was pretty much the same for adding the a/v/ covers.
