[OPEN-ILS-GENERAL] Icon criteria

One thing to keep in mind if you have Playaways and other direct electronic
formats (Form q), the icon will be for e-book or e-audio, which an be
misleading to patrons and staff. Terran created a new icon for PINES for
form q devices and we made sure all the Playaways were correctly coded
(coding for them was not originally q).

There is also a bug where the presence of an 006 for an accompanying CD
triggers the icon for a CD rather than for the print book --
