   Thank you for taking a close look at this. The more eyes that examine it the better. I certainly did miss a few things. Please see my comments in-line below.

Hi Scott, I'm just getting a chance to look these over again and have some more notes (apologies if you've already gotten some or all of this directly):

I noticed the attachments have Mike's name but the project's EIN. Shouldn't the name at the top of all of the attachments should be "The Evergreen Project" to match the main form? Good question. The 1023 Instructions say "Provide your name and Employer Identification Number (EIN) at the top of each page...." I just assumed it was "my" name,  but, now that I think about it, it is probably the entity's name. I will change it.


For Part III we need the locations of the clauses for answers 1 and 2b. (I assume the more the reviewer has to look up themselves the more likely they are to give up and reject it.)  I completely missed this. Regarding III 1, I can't find a Mission Statement in our By-Laws. Does anyone know where I can find that?
Do we need / have anything for 3a? Good question. I think we need to pose this to our attorney.

I updated the Part V 1a attachment to use a table rather than tabbing in the hopes that it's easier to read.  Thank you.

In Part V 5a, I realize we haven't accepted a formal conflict of interest policy yet (though I thought that was the plan at some point?) but were we going to supply answers for 5b and c in that case? We should. I was under the impression that the passage of a COI by the EOB was imminent.

Is an attachment missing for Part VI 1b? There is indeed. I will rectify.

In Part VIII, given that we don't do any of these things currently but might in the future, accepting web donations seems to be one that we should certainly look into someday.

Since this came up during yesterday's call, I thought I'd share the latest version of the 1023 and supporting documents. Schedule G is now blank since we will not be applying as a successor to the SFC. As I indicated yesterday, the only thing missing are the financials. It will be great to have our new attorneys look this over. Please let me know if you have any questions.
