[Evergreen-catalogers] Item template issue in 2.3.4

It is happening on the unified screen accessing this through holdings maintenance and when creating new records.

I also remembered we had one library where another library's prefix would be added the new juvenile DVDs. These were new records using the unified interface to create them. They made a new template and it stopped happening.

I have been busy with other things this morning and had not gotten a chance to look into your question, so glad you were able to get it answered. I agree that people shouldn't be working around bugs, but that is what catalogers tend to do. We often feel as though our concerns are not taken seriously; particularly when we report bugs and they just sit and no one ever addresses them. And then we occasionally hear that developers actively don't think a bug we have reported is important enough to fix. 

I will get it written up for Launchpad as soon as I can.

Received confirmation of it happening the Item Attribute Editor alone.
 To me, this sounds launchpad-worthy, even if it is difficult to reproduce.  Others not on this list may see it and weigh in, if not figure it out.  Since I wrote that part of the code, I wish I had known about it earlier.  Folks really shouldn't be living with and working around bugs.
