[Evergreen-catalogers] Format icons for two formats on same bib record

Janet,

While we don't see the wrong icon problem anymore in 3.0.2, only one icon
displays for those bib records. We have not looked into how we might be
able to get the 2nd icon to display.

While looking into this, I discovered the wrong 2nd icon is displaying for
audiobook records with an 006, so will need to pursue that.
