Creates new format to support audio players (like Playaways)

Format created is “Preloaded Audio” and uses the item type of ‘i’ and item
form of ‘q’

Some discussion about what libraries are doing and would like to see with
new formats.

Is it okay to approve this newly created format and move the discussion of
how to create additional formats and filters to a Cataloging Working Group
forum (and create new LP bug for new requests)?
