[Evergreen-general] OPAC Request a Library Card Form

Thank you for the interest in the form! To answer a few of the questions:

- The form populates a pending patron in the pending patron interface.

- The uploaded images are automatically deleted after a set period of time.

- We had a great deal of discussion about the fact that we have the 
digital images of the ID, and we consulted with our City's lawyer about 
this. In the end, we went forward with it, recognizing that patron 
interaction with the form is a type of implied consent for us having 
access to these images. We only keep the images for a set period of 
time, and there are two levels of security for staff to be able to 
access them. We also offer other options for obtaining a library card if 
a patron is uncomfortable uploading their images with us.

I hope this helps! So far the form has been working very well for us.
