[Evergreen-general] Cover Image uploader

NC Cardinal recently upgraded to 3.9.1. Since the upgrade, we have discovered that the custom cover image uploader isn't working. As described by one staff member struggling with it: "If it works, it works instantly. Clear your cache and look quickly; a massively compressed version of it will be there. Shortly thereafter, the image vanishes from the server and you're back to gray squares."

Essentially, what's happening is that people are uploading images. Sometimes they see the cover image instantly, most times they don't. Sometimes one that was visible instantly no longer shows up for them later. Sometimes a hard refresh will force the image to show up, sometimes it doesn't. Sometimes it takes more than 24 hours for the image to be visible.

I've tested it myself several times and found no consistent pattern.

Has anyone else run into this issue?
