Hi John:
I can think of at least a couple issues that may cause this.  If you can
zip up your apache configs and logs, it will most definitely help.
 In the past, somebody came out of the woodwork to report a
configuration issue that all the "usual" troubleshooters overlooked.
That may be the case this time.
