[OPEN-ILS-DEV] Setting default of 2 on asset.copy.loan_duration and asset.copy.fine_level

Since 'normal' is a sane default, this patch sets the defaults for
asset.copy.loan_duration and asset.copy.fine_level.

It also adds brief comments explicating what the integer values mean.
