Yes, it's pretty insane..
Talking to several users of the big 3, I've gotten the sense that if you're a long time customer - and you don't have many of these add-ons already purchased from the first contract, then you're on the purchase train every time you need to do something new. But if you're a new customer and if the vendor wants to sweeten the deal especially now with increased competition, then you'll get thrown into your contract a bunch of "add ons" that other similar customers have to pay. That's another reason for so much of the "confidentiality" requirements in these deals: nobody is supposed to know what constitutes a "base ILS" and what the other library is paying for the base toolbox.
So new customers can get a sweet full package deal (depending...), and the older ones get to keep buying pieces to maintain basic functionality...
