[OPEN-ILS-DEV] Staff Client Errors

Hi Everyone,
I have just done a fresh install of Evergreen 1.4.0.6 and everything seemed
to be working fine except for the fact that I am having trouble logging into
the staff client the error that I keep receiving is:

Network or server failure.  Please check your Internet connection to
192.168.1.3 and choose Retry Network.  If you need to enter Offline
Mode, choose Ignore Errors in this and subsequent dialogs.  If you
believe this error is due to a bug in Evergreen and not network
problems, please contact your help desk or friendly Evergreen
administrators, and give them this information:

STATUS:

if i click ignore errors I get this error:

We experienced a network/server communication failure.  Please check
your internet connection and try this action again.  Repeated failures
may require attention from your local IT staff or your friendly
Evergreen developers.

The staff client failed to retrieve expected data from this call,
"open-ils.circ.survey.retrieve.required"

Any Insights would be appreciated
