For the short term, I've committed 15292 to trunk (15294 in rel_1_6_0)
to remove the non-functional serial MFHD code from O:A:Ingest, and also
changed the README in Open-ILS/tests/datasets to drop the
direct_ingest.pl step for MFHD records and note the current need to
manually supply an owning org_unit for each record before loading into
the database.
