[OPEN-ILS-DEV] ***SPAM*** Change to serial schema

After some discussion in IRC on Friday between Mike Rylander and Dan Wells, Mike committed some further changes to the serial schema.  Summary:

1. Make caption_and_pattern a child of subscription rather than a child of serial.record_entry.

2. Make distribution a child of serial.record_entry (it is also still a child of subscription and several other tables).

3. Make subscription a child of biblio.record_entry instead of a child of serial.record_entry.
