[OPEN-ILS-DOCUMENTATION] Upgrade Notes for 1.6

I am not a Linux nor an Evergreen expert, so I would feel better if others looked at it, too.  If anyone has gone through the process, could you take a look at my notes?  Let me know of if I am off on something.  I would hate to give someone the wrong information.
