[OPEN-ILS-DOCUMENTATION] Serials Creating a Special Issue

I didn't see anything for this so I wrote up some instructions here.  I
don't know how to get it into the format for the repository if the community
wants to included it.  It also might be worthwhile for someone else who
works with serials to double check that it is correct.
