Grant, could you follow the instructions here
http://open-ils.org/dokuwiki/doku.php?id=scratchpad:debug_console to
enable a debug window for your staff client, and then send the content
of that window to me (or us, if you don't mind folks possibly seeing
the password for your test user) after you cause an error?
