[OPEN-ILS-GENERAL] Copy Locations Editor (v.1.4.0.2)

I'm logged in as system administrator and I would like to edit our copy
locations, I can enter names and check holdable, circulate etc. but the
Create button is greyed - I cannot actually enter my changes.  Any ideas?
Is this a problem with our system? or is this perhaps a permissions thing?
I'd think as system administrator I should be able to make any changes??
Any ideas as to what is causing my trouble?
