Thanks Thomas based on your explanation we've got it working!

I had to turn the flag for circ.holds.usr_not_requestor in 
config.internal_flag to FALSE

Jenny, Wonder if the DIG folks need to document this for < 2.1

Also thanks to Ben Shum for helping tackle the issue!
