Thanks for the information. We haven't pulled from the master repository
recently, which may be the issue.
My Account Preferences-->Search Preferences page has nothing but "USER
SETTINGS" statement and the commented out code from the default OPAC
screen. I can uncomment it, but it shows no functionality. We will be
doing a fresh load to the production server shortly, which I hope will
correct this.

This is working for me with a completely fresh checkout of
template-toolkit-opac and a subsequent pull from the Evergreen master
repository as of today (using the default MARC item_type values from a
completely clean database schema creation).

The Checkout History opt-in is working for me, although I'm sure people
have different ideas of what "working" means. If I opt-in under "Account
Preferences -> Search Preferences -> Keep History of Checked Out Items",
then I can see my circ history in "Items Checked Out -> Check Out
History"; if I opt-out, then I can't.
