David, 

You are right, my request is to help us better handle the exceptions.  We, for example, have set up our current program to always create a item record at the time of ordering.   For various reasons (mostly to do with media kits) we do order items where we don't want a new holding/item record created.  Because we don't have a  line option we either set up a new PO for those few items, or remove the item record after the item is received.  Both of which cause extra work for staff.  I imagine that each library would have their own policies on how they want to handle this type of situation, but I think flexibility is the key.

I suspect that this will be a policy decision rather than something
that staff will think about when selecting or ordering materials.

That is, we either always create a single stub bib record for the
title, or we always create a bib record and associated item records.
