One feature of a survey over a stat cat is that you can record
responses multiple times.

E.g. How was satisfied are you with your library service today?

In early incarnations of Evergreen, we'd also expose specially-flagged
surveys on the public catalog to authenticated users.  With a bit of
work, this feature could be re-implemented.
