I think this may be from an improper shutdown of Evergreen.  I'd go
through the steps here:
http://evergreen-ils.org/dokuwiki/doku.php?id=troubleshooting:checking_for_errors

The main steps are stopping services, clearing out /openils/var/run/,
and starting services.
