[OPEN-ILS-GENERAL] Report to count Library Card Creation by Staff Login or Workstation

Deanna, Jennifer, and all,

The lack of the staff user that created a patron and the workstation at
which the patron was created is a problem for us as well.

There is a Launchpad bug open on this issue:

https://bugs.launchpad.net/evergreen/+bug/1607827

I am working on a fix for this, but have more work to do. Feel free to add
your comments to the Launchpad bug to add some "heat" to it.
